Meibomian gland dysfunction (MGD)

Etiology

Meibomian glands are the sebaceous glands located in lid margin. The secretions from these gland is lipid(Oily) which is slowly released in tears which helps the evaporation of water from the tears , thus helping to prevent dry eyes

Failure of these glands to produce or secrete oil - due to chronic blockage, thickening of the meibum, etc. will affect the quality and stability of the tear film, which in turn will produce classic dry eye symptoms

Meibomian glad dysfunction is very common and unfortunately it often goes undiagnosed. Even when diagnosed is often not treated or is not treated effectively until it has become chronic or severe.

Confussion

Many a times MGD is misdiagnosed as Dry Eye and is treated wrongly –because MGD mimic the symptoms of Dry Eye, but dry eye is aqueous deficiency and not lipid in the tear composition

Confussion

  • Stickiness
  • Pricking or foreign body sensation
  • Swelling on lid margins
  • Redness specially at lid margins
  • Itching
  • Irritation

Treatment

Treatment includes, depending on the severity of the condition

  • Warm compresses or Hot fomentation (to liquefy any secretions that have solidified and are clogging the glands)
  • Lid scrubs
  • Oral antibiotics (such as doxycycline)
  • Omega 3 supplementation
  • Topical antibiotic ointments
